Just type on each line and answer the questions.
On the very last page, on the bottom there will be a button that says SUBMIT. Just click on that button when you are done and it will save the pdf. Just send us an email and attach the pdf file you just saved.
You will need to have Adobe Acrobat Reader 7.0 or later installed on your machine. If you do not have Adobe Acrobat Reader or have an older version, you can download a new version here for free!

A copy of your currently valued loss runs for the last 3 years (this is a report from your current carrier for the last 3 years track record that will state if you have had any claims or not, similar to your credit history). You will need to request this from your current agent now as it sometimes takes them a while to respond! |

You may submit on your letterhead, a statement stating: (field name) has had no claims in the last 3 years and we are waiting for our loss runs from our current carrier to start the process. |

A copy of your current waiver |

A drawing or map of your field layout showing entrance, field location, any pro shops, streets, etc. |
